Our Primary Care Pain Support service was integrated into Sevenfields Primary Care Network, pairing Musculoskeletal First Contact Practitioners with Pain Specialist Health and Wellbeing Coaches to provide personalised care and education.
Over 12 months, the programme supported 380 patients, with 74% avoiding GP visits during the programme and achieving a remarkable 42% improvement in self-efficacy scores.
By integrating tailored interventions, remote appointments, and community-based Pain Cafés, this model not only reduced GP workloads but also empowered patients to take control of their health.
