Data Support and Clinical Outcomes
As part of the service to the Primary Care Networks we work with, we provide monthly key data on the clinical services we deliver for them. This monthly data is delivered at practice and PCN level, demonstrating clinical activity, patient feedback, outcomes and facilitates service development opportunities.
Demonstrating FCP Impact
We understand that Primary Care Networks need to demonstrate the impact of all FCP roles. To help you, we collate, review, analyse and send data back to PCN and practices, including patient feedback, activity levels, outcomes, critical instances and service development opportunities as part of the service we give you.
We do this so you don’t have to, freeing you to do more in your practice. We give you more time – allowing you to run your practice, not collate data.

Why do we provide this data?
- Referral levels: To give clarity on utilisation and ensure effective diary management.
- DNA levels: We typically maintain a DNA (Did Not Attend) rate under 9.4%. If the DNA rate exceeds 10%, we flag it internally to proactively manage the service and implement steps to address the issue with the practice.
- Number of follow ups: We aim for less than 10% follow-ups to avoid a waiting list, maximising availability for new patients.
- Referral source (Reception Team or GP): Guides training requirements for the PCN and local practice teams.
- Referral per practice: Ensures equitable usage across a PCN/organisation.
- Age range of patients and affected body area: This data guides our MSK training for all staff and the PCN/organisation.
- Critical incidents or red flags: Identifying these ensures joint learning and oversight.

Outcome of FCP consultations:
- 1. GP involvement
- 2. Onward referral
- 3. Referral for imaging, injection and medication review
- Number and destination of onward referrals into existing MSK pathway and secondary care: This allows us to evaluate referrals, monitor ongoing treatments, and assess the impact on secondary care. Typically, the number of referrals to secondary care decreases, but the actions taken once patients arrive in secondary care increase.
